As the OBRA, PASRR Coordinator for Centennial, I receive and process all requests for Pre-Admission Level II evaluations for nursing home placements. I also work closely with the 10 county nursing homes social services departments to review any changes in psychotropic medications or psychiatric diagnosis for nursing home residents. I assist in making sure that the resident is assigned a therapist for intake/counseling and/or is scheduled to see our psychiatric medical staff for medication management if appropriate. As an R.N., I am available to the psychiatric medical staff at Centennial for assistance in refilling medications through the pharmacies, completing patient assistance application for medications and distributing samples of medication to clients when appropriate. I have been trained to assist clients of Centennial with applying for Social Security benefits through the SOAR program.
As a member of the Clinical Team, I serve on Centennial's internal Quality Improvement Team. We review clinical records for accuracy and completeness.
I am also involved with the development of Integrated Care Services and Wellness programs for Centennial.
