FOURTH STREET RESIDENCE

Fourth Street Residence is a HUD certified residence where residents pay rent based on their income. It has eleven beds and serves people with severe and persistent mental illness. Fourth Street is staffed at quarter time to provide supervision to aid residents in meeting their own self care needs by assisting them with daily living skills and medication management. Regular house meetings help resident’s problem solve and address interpersonal growth. Residents are encouraged to stay a minimum of one year, however there is no required length of stay. Individual treatment plans are geared towards helping the resident be as self reliant as possible.

Population Served

Adults with SPMI who need support to function in an independent living environment.

Services Offered:

Ten apartments available to rent at below market rates. Assistance with gaining independent living skills, case management, and a supportive peer community.

Qualifications:

1. Needs more intense monitoring
2. Must be 18 years of age
3. Carry a primary mental health diagnosis
4. Will not be considered for housing if primary diagnosis is drug or alcohol related. Needs to be in remission at least 2 years.
5. IQ at or above 60
6. Have no incontinence issues (bowel or bladder) unless resident is capable of self-care
7. Have no communicable diseases unless the resident is receiving medical or drug treatment
8. Have no reported violent behavior
9. No felony convictions in the past seven (7) years
10. Voluntary admittance, except on a case by case review
11. Must be ambulatory
12. Must meet section 8 income requirements
