COMMUNITY-BASED MANAGEMENT PILOT PROGRAM

The target population for the Pilot Program includes residents of Logan County who are (1) less than 18 years of age, and (2) charged with or adjudicated for an offense or committed to the Colorado Department of Human Services, and (3) diagnosed by a mental health professional as having a serious mental illness. Individuals adjudicated for or convicted of a Class 1 Felony or sexual assault will be excluded from the program. Because the treatment protocol for the Pilot Program includes procedures based on the principles of cognitive behavior therapy, which requires a criterion level of intellectual functioning, individuals with mental retardation also will be excluded.

The Pilot Program will include services for 15 juveniles during year one of the program (January 1, 2001-June 30, 2001). During years two and three, contingent on continued funding of the program, 22 and 30 consumers will be served, respectively.

The Pilot Program is the most inclusive, treatment-based program available. The continual evaluation of the consumer, family, service planning, and treatment, guarantees multi-level success.
