







| | The following is a general overview of the insurance benefits that Centennial Mental Health Center offers its employees. For complete information on each benefit, please refer to the Employee Handbook. - LIFE INSURANCE (.75 FTE): An employee (3/4 time or more) will be covered at the Center's expense under a group term life insurance policy for which the benefit is twice the employee's annual salary up to $150,000. Life insurance in excess of $50,000 is taxable once a year on the final payroll run for the year.

- MEDICAL/DENTAL INSURANCE (.75 FTE): An employee (3/4 time or more) will be covered by the health insurance offered by Colorado Employers Benefit Trust (CEBT). The employee may choose a higher benefit option or enroll dependents at his or her own cost. A dental plan is available at the employee’s cost. The employee (and dependents, if enrolled) are covered on the first of the month following 30 days of employment with CMHC.

CAFETERIA PLAN (.75 FTE) IRS Section 125 (cafeteria plan) is a tax regulation that lets employees redirect a portion of their taxable gross salary to one of three pre-taxed benefits accounts to pay for eligible expenses: Out-Of-Pocket Medical Expenses Child or Dependent Day Care Expenses Employer Sponsored Insurance Premiums (Health, Dental, Life, etc.) ADDITIONAL AFLAC INSURANCES (.75 FTE) An employee (3/4 time or more) has the option to receive addition insurance including: Accident, Cancer, Intensive Care, and Life.
