Reports to: Deputy Director

Type: Exempt 

Position Summary: Assures appropriate level of care for Center consumers through effective management of utilization of high-intensity treatment services. Supervises the clinical services provided by the Center in counties as assigned by Deputy and Executive Director.  Insures that (1) programs and services provided by the Center comply fully with the standards, rules and regulations of applicable regulatory and payor agencies, (2) programs and services offered by the Center are consumer-driven and consistent with the Center’s mission and policies, and (3) direct service staff adhere to the highest standards of clinical care.

Essential Duties and Responsibilities: 

1.   Tracks mental health and substance abuse admissions and discharges for in-patient, acute treatment unit (ATU) and residential treatment center (RTC) settings.

2.   Initiates discharge planning contacts with in-patient, ATU and RTC facilities at time of admissions.

3.   Monitors status of in-patients.

4.   Coordinates discharge planning and follow up services between institutions, community service agencies, consumers, families of consumers and assigned clinical staff.

5.   Coordinates service integration between Center programs and other agencies.

6.   Sits on the Centennial Quality Improvement Committee and implements the quality assurance, quality improvement and utilization review efforts of the Center.

7.   With the assistance of the Quality Improvement Committee, determines staff training needs and develops and coordinates professional development activities for center-wide clinical and support staff , including all annual and on-going required training.

8.   Monitors and evaluates programs for quality of care and cost effectiveness. Reports findings and recommendations to the Executive Director.

9.   Continuously monitors and refines processes for documenting and reporting the clinical services provided by staff.

10.  Performs other duties as assigned.

Job Requirements: 

EDUCATION

Master's Degree In psychology, social work, or other human services field.

CERTIFICATES, LICENSES, REGISTRATIONS

Licensed or licensable in the State of Colorado as a Licensed Clinical Social Worker, Marriage and Family Therapist, or Psychologist.

EXPERIENCE

Direct service and/or administrative experience in transitioning consumers from inpatient and residential settings to community settings. Demonstrated experience in quality assurance standards and QI implementation.

ABILITIES, KNOWLEDGE, SKILLS

Extensive knowledge of mental health, substance abuse and hospital systems.  Knowledge of community resources for managing consumers in transition, including ability to develop and oversee community-based agency and consumer resources.  Ability to serve wide range of consumers, including culturally diverse populations.  Knowledge of cultural issues that may have a bearing on service provision.  Excellent written and verbal communication skills.  Computer literacy to include basic skills in e-mail communication and word processing.  Ability to travel within the Center's service area or to other locations as needed.
