Reports to: Community Support Program Manager Type: Non-Exempt Position Summary: Provides a variety of services that include a continuum of high to low intensity services designed to improve or maintain a consumer's abilities to function in the community. Essential Duties and Responsibilities: 1. Assists case managers and therapists in providing case management to consumers, including intakes, functional assessments and service delivery. 2. Works with the consumer to access appropriate community resources as needed to maintain ability to remain in the community. 3. Co-leads skills training classes in a variety of areas relative to basic life skills for small groups of individuals. 4. Provides direct skills training on a one-to-one basis. 5. Transports consumers as needed to access services. 6. Works with consumers in residential settings to ensure cleanliness of living quarters and meal preparation. 7. Maintains accurate and timely clinical records consistent with Center standards. 8. Participates in meetings, in-service and supervisory sessions as required. 9. Performs other job duties as assigned. Job Requirements: Abilities, Knowledge, Skills: Ability to work well with special population consumers. Effective written and verbal communication skills. Ability to work flexible hours, which may be required. Ability to travel within the Center's service area or to other locations as needed. |
