POSITION SUMMARY
Provides monitoring and supervision of clients admitted to the Crisis Respite Program. Will respond to requests from Emergency Response Team for monitoring of clients, working 12 hour shifts at a designated Respite facility location. When indicated, the Respite Staff will work in teams of two to provide adequate client supervision and support.
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Provide supervision/monitoring at a designated Respite facility location for individuals who have been assessed by Centennial ERT or clinical staff and have met the criteria for a Respite level of care.
2. When necessary provide “ride along” supervision during transport by Centennial ERT or clinical staff to or from facility, OP office, or other activities appropriate to the Respite program. This may also include walking with client short distances to an office or other location.
3. Assist in cooking and providing meals for residents of the Respite facility as needed.
4. Monitor cleanliness and upkeep of the Respite facility per the guidelines specified for that location.
5. Provide supportive companionship and utilize MHFA and other supportive counseling skills for residents in the Respite program.
6. Communicate as pre-determined in each case with Centennial ERT and clinical staff regarding client’s status and Respite Plan of Care.
7. Help facilitate the Plan of Care with the Respite client, under direction of ERT and clinical staff. This may include assisting with case-management activities such as referral and linkage to community resources.
8. Monitor client self-administered medications and log same.
9. Keep a summary log of all pertinent activities that transpire during the Respite stay.

• Administers/maintains Housing Choice Vouchers, State Housing Vouchers, Fourth Street House and related services or subsidies.

POSITION SUMMARY: Oversees delivery of behavioral health case management services in outpatient clinic(s). The Program Manager is responsible for implementation of various case management programs, including but not limited to ACT (Assertive Community Treatment) program, Journey (Day Treatment for Adults), and Medication-Only services. The Program Manager provides direct supervision and training of Case Managers, Peer Specialists, and mentors as part of the leadership team collaborates with other service providers to provide high quality comprehensive client care.
CORE RESPONSIBILITIES
- Oversee provision of case management services to clients, including individual services, psychoeducational groups and rehabilitation activities; implement referral, assessment, and service planning processes; facilitate collaboration among case management/peer staff and other agency and community service providers

Position Summary:
We are seeking an experienced Systems Administrator who is eager to use and grow your technological skills. In this role, the successful individual will protect our critical information, systems, and assets, build solutions, implement new software and hardware, provide training, and assist in the development of an overall IT strategy. From early on, you will be given challenging assignments, lead initiatives, and take ownership and responsibility. You will also receive self-paced training to help you become a leader in your field. Essential Duties and Responsibilities:
Install, configure, update, and maintain Microsoft Windows Servers and Linux servers (both physical and virtual) and the applications those servers run.
Coordinate with the Information Technology Director to ensure availability and reliability of servers to meet business demands.
Ensure that periodic on-site and off-site backups of critical data is accomplished according to procedures.
Write and follow procedures to keep the servers running efficiently. This includes monitoring resources to identify potential overload conditions, failed/failing equipment, and resolving bottlenecks.
