Request for Proposals

Centennial Mental Health Center is proud to sponsor a Mini-Grant Program aimed at acknowledging and supporting efforts that advance awareness of mental health and addiction issues in our local communities. Contingent upon available funding, a series of awards will be issued.

Responsibility for Mental Health wellness is shared equally by each of us through our schools, agencies, businesses, and most of all, our families. The Mini Grants will offer financial support to any of these entities submitting a plan with mental health or substance abuse awareness as its goal. Proposals are encouraged to be written collaboratively with others, but can be submitted by a single applicant as well. There is no age restriction on either the target population included in the proposal or the grantee.

Proposals may address a wide variety of projects, including but not limited to, individual and group sponsored activities, purchases of materials and equipment promoting mental health or substance abuse awareness, underage alcohol use, suicide awareness and prevention, and educational and training opportunities. Reporting requirements will be minimal and will involve a brief summary of activities and successes sponsored through the grant.

Proposals may request up to $500.00 and may be submitted from any of the ten counties served by Centennial Mental Health Center, including Elbert, Lincoln, Kit Carson, Cheyenne, Yuma, Washington, Phillips, Sedgwick, Logan, and Morgan counties.

Criteria for rating

40 Points - Relevance to mental health and / or addiction awareness

30 Points - Evidence of collaborative partnerships

20 Points - Evidence of measurable outcomes

10 Points - Budget & budget narrative

Proposal narratives must include the following

  • Name of person(s) or agency applying for grant (include contact information),
  • Population to be served,
  • Goals and objectives, including how success of the project will be measured,
  • Resources needed,
  • Amount requested, and
  • Budget and budget narrative.
